Advance Care Planning for Faith Communities: A Quality Improvement Project

Program Description:
This presentation will provide an overview of a quality improvement project regarding advance care planning in faith communities. An advance care planning training tool will be reviewed and disseminated for the faith community nurse. This will enable the faith community nurse to be able to discuss advance care planning with clients.

Lori Lee Hacker, DNP(c), MSN-Ed, NP-C, RN, CNE. She has been a Palliative & Hospice Nurse Practitioner for 5 years, Post-Secondary Nursing Faculty for 15 years, and will complete a Doctor of Nursing Practice in Leadership in March 2021 with a focus on Advance Care Planning in Faith Communities as a Quality Improvement Project. She is currently Assistant Professor, Marian University
This activity has been submitted to Alabama State Nurses Association for approval to award 1.0 contact hour. Alabama State Nurses Association is accredited as an approver of continuing nursing professional development by the American Nurses Credentialing Center’s Commission on Accreditation.
